This policy describes how Strategic Decision Review (SDR) handles information submitted through MyDecisionField.com. This website is an informational and inquiry website, not a client records portal.

If you send an inquiry, we collect the name, email address and brief message you choose to provide. Our website hosting provider may also process routine technical information, such as IP addresses and request logs, as part of serving and protecting the site.

How information is used

We use inquiry information to review and respond to your message, manage follow-up correspondence, and maintain the security and operation of the website. Form submissions are processed and may be stored by Netlify, our website hosting and form service provider, and may be delivered through email notifications. We do not sell inquiry information.

Sensitive information

Do not submit patient-identifiable information, medical records, tax returns, financial account or Social Security numbers, passwords, or confidential legal materials through the public form. The inquiry form is not intended for protected health information or other sensitive records. SDR’s mapping service requests facts, not underlying files. Do not send underlying documents or clinical records.

Sharing and retention

Information may be accessible to service providers supporting website hosting, inquiry processing or communications, and may be disclosed when required by law. We keep inquiry records as long as reasonably necessary for responding, business records, security or legal obligations, then delete them according to applicable practices and requirements.

Your questions and choices

You may ask about the information you submitted or request that it be corrected or deleted, subject to applicable recordkeeping or legal requirements, by contacting hello@MyDecisionField.com. We may need to verify a request before acting.

Links to outside sites are governed by those sites’ own policies. We may revise this policy as website functions or data practices change, and will update the effective date above.
